WAIVER, RELEASE AND ASSUMPTION OF RISK: In consideration of my participation in Liga Union Deportes LLC, I hereby waive, release and discharge all claims for damages for death, personal injury, or property damage which may occur as a result of my participation in Liga Unión Deportes LLC. This release discharges in advance the Friends Select School & Liga Union Deportes LLC, its owners, officers, agents, servants, and employees, game officials and referees.I understand that this WAIVER, RELEASE and ASSUMPTION OF RISK form will remain on file with the organization and that it will apply to the current event as well as my participation in future events.I certify that there are no medical condition that would cause participation to increase the risk of hazard to my health. In addition, I authorize the I hereby authorize the use of my photo/video and/or information related to my experiences with Liga Union Deportes LLC. I understand and am agreeing to all costs associated with myself and teams participating in the LU Sports Events. I understand this information may be used in publications, including electronic publications, audiovisual presentations, promotional literature, advertising, community presentations, and media and/or other similar ways.
